What is the standard knife used by the US Army? Fixed blade, folding, etc?
Is it issued or does the soldier have a choice?
It depends on the service branch and also sometimes individual unit as well. it's always a fixed blade.
Soldiers also have a choice to have their own knife (knives) as well.

Not much of a knife if you ask me, in terms of edge holding, but it's tough and works as a bayonet which is primarily a stabbing weapon anyway.
USMC likes Kabars more.
USAF issues Ontario knives (similar to Kabars) to the pilots.
Seals and other special ops units get treated better. I know several occasions when the were issued semi-custom knives.
As for the individual soldiers, there is no restriction on additional knives. Standard issue is given, and then you can have your own stuff as well. However, from all the email exchanges and posts on knife related forums I've seen from soldiers in various services, most of them prefer to use standard issue, plus a multi-tool like Letherman or Gerber.
Expensive customs sit in the lockers most of the time, although there are exceptions to that as well.
